Transition Mapping Worksheet

Purpose: Navigate Change Without Losing Stability

Transitions disrupt more than schedules. They disrupt identity, rhythm, and certainty.

Whether you are changing roles, relationships, environments, or expectations —clarity reduces unnecessary anxiety.

This worksheet helps you map the shift instead of reacting to it.

What Makes Transitions Difficult

Transitions often involve:

• Loss of familiarity

• Shifting expectations

• Unclear identity

• Temporary instability

• Grief (even when the change is positive)


Even good changes require recalibration.


Step 1: Name the Transition Clearly

Write:

What is changing?

Be specific.

Instead of:“I feel off.”

Try:“I am moving from ______ to ______.”

Clarity reduces emotional vagueness.


Step 2: Identify What Is Ending

Every transition contains an ending.

Ask:

• What routines are ending?

• What identity roles are shifting?

• What comforts am I losing?

• What expectations no longer apply?


Unacknowledged endings create lingering tension.

Step 3: Identify What Is Emerging

Now ask:

• What new opportunities exist?

• What new expectations are forming?

• What version of me is required here?

• What skills will matter most in this next season?


Transitions require adaptation, not reinvention.


Step 4: Separate Fear from Fact

Write two columns:

Facts:What is objectively true?

Fears:What am I imagining or predicting?

Transitions amplify projection.Separating fact from fear restores proportion.


Step 5: Define Stability Anchors

Ask:

• What will stay consistent?

• What values remain unchanged?

• What routines can I preserve?

• Who remains steady in my life?


Stability anchors reduce emotional drift.


Step 6: Choose a First Stabilizing Action

Not a full plan.Not a five-step overhaul.

Just one stabilizing move.

Examples:

• Establish a morning routine

• Clarify expectations with one conversation

• Set one boundary

• Schedule one check-in


Momentum builds confidence.


Reflection Prompt

When you think about this transition:

• Does it feel like loss, growth, or both?

• What part of this change feels most destabilizing?

• What part quietly feels right?


Most transitions contain both grief and expansion.

Mapping:

• Reduces ambiguity

• Clarifies controllables

• Stabilizes identity
